Payments Online

You may securely pay online through Square for an appointment, paperwork, or other items or services by clicking the links below. Please enter information as requested which will assist us in applying the payment correctly. If you wish, you may provide your credit card number, expiration date, and mailing zip code through our patient portal. We will destroy that information as soon as we manually process the transaction.
